The portal represents the digital operations layer supporting the LTOP partner program with FPG Mercantile representation, member onboarding, official notices, payment visibility, document coordination, and insurer handoff support.
The website provides digital tools for customer registration, identity and document intake, application tracking, payment status visibility, receipt and export requests, insurer handoff files, proof-of-coverage updates, and service coordination for MDMIP insurance applications.
The platform provider is not the insurer, insurance underwriter, payment institution, policy issuer, or final claims decision-maker unless a separate official appointment says otherwise. Insurance coverage, approval, policy terms, exclusions, benefits, and claims decisions remain subject to the authorized insurer, official policy documents, and applicable law.
Applicants are responsible for providing accurate and complete information. Incorrect, incomplete, outdated, or unverifiable information may delay review, payment matching, certificate issuance, servicing, or claims assistance.
Registration alone does not activate insurance coverage. Coverage is subject to eligibility review, payment verification, document validation, certificate issuance, and official activation under the approved policy terms. After verified payment, processing and activation may take around 24 to 72 hours depending on cutoff timing, completeness of records, provider confirmation, and insurer review.
